Complete blood count (CBC) is a very common diagnostic test.
It is also called as Haemogram, Full Blood Count (FBC), or Full Blood Exam
(FBE) and is performed for various reasons.
Parameters used in CBC
- Haemoglobin
- Red blood cell (RBC) count
- Platelets or thrombocyte count
- Mean platelet volume (MPV)
- differential counts of white blood cells (WBC)
- RBC indices such as mean corpuscular volume (MCV), mean corpuscular haemoglobin (MCH), mean corpuscular haemoglobin concentration (MCHC), reticulocyte count, Red blood cell Distribution Width (RDW), and haematocrit (HCT)
Hematocrit (HCT, packed cell volume, PCV) measures the amount of space (volume) red blood cells take up in the blood. The value is given as a percentage of red blood cells in a volume of blood.
MCV shows the size of the red blood cells.
MCH value is the amount of hemoglobin in an average red blood cell.
MCHC measures the concentration of haemoglobin in an average red blood cell.
Red cell distribution width (RDW) can also be measured which shows if the cells are all the same or different sizes or shapes.
Reticulocyte count is the number of young RBCs.
Mean platelet volume (MPV) measures the average amount (volume) of platelets. Mean platelet volume is used along with platelet count to diagnose some diseases. If the platelet count is normal, the mean platelet volume can still be too high or too low.
The above table indicates normal value range for adult male and female. Normal value ranges differ according to age. Very small deviation from normal values are considered to be normal.
Interpretation of CBC Reports
Now we will learn how to use the above parameters in order to determine abnormalities. Small changes are often seen in our body and so small deviations from the normal value range must be considered normal.
High RBC count (condition called Polycythemia) may be due to
- Increased RBC production
- Some kidney disorder that causes excessive secretion of hormone Erythropoietin (that stimulates RBC production)
- Low oxygen levels in blood for prolonged periods, living in hilly or oxygen deficient areas (in this case, there is nothing to worry)
High Haematocrit values may also be because of dehydration (since the fluid content of blood goes down in this case) along with the ones responsible for high RBC count.
Low RBC count (condition called Erythrocytopenia) and low haematocrit values are indicatives of Anaemia. Malaria, Haemorrhage (excessive bleeding), Chronic Kidney Disease and other conditions in which abnormaly high number of RBCs die in a short time span or RBC production is significant reduced (ex. deficiency of vitamin B12, folic acid) leads to Anaemia.
Low WBC counts hint to a weak immune system which may be due to
- Certain autoimmune disorder
- Infection of liver, spleen
- Less production of WBC 's because of some abnormality in white bone marrow
- HIV or some other pathogen that attacks WBC's
Slight increase or small increase in WBC count is called Leukocytosis. This is mainly caused in response to entry of infectious agents in our body. Other causes may include
- Abnormality in white bone marrow
- Inflammatory diseases like Rheumatoid arthritis
- Damage to tissues
- Physical or emotional stress
Increase in Eosinophil count (called Eosinophilia) is seen
in case of allergy. Large increase in WBC count is called Leukaemia.
